Parents of our Medicaid CMO Patients,
This is a friendly reminder that the Annual Anniversary Period for Medicaid CMO members is right around the corner! Historically, Medicaid distributes information packets between late March and early April. These packets typically contain documents describing what the Annual Anniversary Period is, the options available to you, and enrollment and CMO election forms. Historically, plan changes go into effect June 1.
Your options
- CareSource – Must be the CMO plan (Medicaid/PeachCare for Kids)
- PSHP
What you need to do: If you wish to change your plan, you need to complete the provided paperwork and submit to the Georgia Department of Community Health (DCH) by the deadline defined in the info packet.
IMPORTANT: Please ensure you list a Peachtree Park Pediatrics provider as your PCP. If you don’t, one will be randomly assigned to you and we will not be able to see your child.
Even if you do not wish to change plans, we recommend submitting the paperwork with one of our providers listed as PCP to ensure one isn’t randomly assigned anyway.
Please note that we do not have access to the actual documentation provided by DCH. As a result, the dates and instructions provided here are based on historical information. Please refer to the documentation provided in your information packet.
