Please Note: The easiest way to access your form, once completed, is through the Follow My Health patient portal. To have your completed form uploaded to the portal, please select ‘Upload’ in the ‘To Be Received By’ field in the form below. If you are not yet signed up, click here to submit an invitation request!

1. Call the office to pay your annual $10/Child Administrative Forms Fee

2. Upload Your Form below or email it to Forms@PeachtreeParkPeds.com. Please indicate whether you would like us to mail or upload your form to the patient portal, or if you would prefer to pick it up in the office.

3. If you do not have a patient portal account and would like your form uploaded, please click here, to fill out an invitation request. You will receive an email from noreply@followmyhealth.com in the next 2 business days with step by step directions on how to register.
