Annual Check-Ups
It’s time to think about your child’s annual check-up… appointments fill up fast over the summer! Many sports programs require the check-up to be within a certain period of time, so check your school or camp’s requirements.
It’s also FORMS season!
Forms are completed in 3 business days. If you request completion of a form prior to 3 business days, there will be a $25 charge to expedite completion. Forms generally cannot be completed “on the spot”
You now have the option to upload your form and have it scanned to your child’s portal account. You will find this option under the “Forms and Handouts” page.
As a reminder, we now have a $10 per child annual administrative fee. You may pay this at your child’s check-up, or if you just have forms, you can call us with a credit card payment.
